What To Expect

Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. It will also help alleviate any unnecessary anxiety you may be feeling prior to your first appointment. Educate yourself on your symptoms by reviewing the content on this website. Also, take some time to review our staff page and familiarize yourself with the doctors. We look forward to your first visit.


New Patients should bring the following:

  • Your insurance card
  • Physician referral forms if required by your insurance provider
  • A list of current medications
  • Medical History information
  • Any recent labwork or relevant test results. (ie. X-ray reports, PSA reports, etc.)

If you have any questions regarding this information, please call our office and our helpful staff will assist you in what to bring to your initial consult.


Initial Visit:

When you arrive for your first appointment, you will receive a thorough evaluation of your health history and a physical. Once we obtain this initial information, we will work with you to develop a comprehensive treatment plan and provide you with the information you need to make sometimes difficult treatment decisions. Our staff is committed to individualized patient care and discretion in all patient concerns.


Insurance and Billing

We accept most private health plans, see below for a list. If you do not see your plan, or have any questions, please feel free to contact our office. We also accept Cash paying patients, and are happy to work out a payment plan for you. Most charity care plans are accepted.
